High-energy pilon fractures management

چکیده

Pilon fractures are distal tibia end fractures affecting the weight-bearing articular surface. They represent around 1% of all lower extremity fractures, and are more common in males between 35 and 40 years of age.1 Such injuries may be caused by highor low-energy mechanisms. Axial compression injuries where the talus impacts on the distal articular surface, causing implosion of the tibial plafond are caused by high-energy mechanisms with key characterisitcs including severe soft-tissue injuries, comminution and multiple displaced articular fragments. Conversely, rotational injuries are typically low-energy fractures with less soft-tissue injury and, usually, a distal tibial spiral fracture pattern. In this review, we will focus on the management of high-energy, axial compression injuries.

Management of high-energy tibial pilon fractures

Tibial pilon fractures result from high-energy trauma unlike usual ankle fractures. Their management provides numerous challenges to the orthopaedic surgeon including obtaining anatomic reduction of articular surface and the management of associated soft tissue injuries. This article aims to review major advances and principles that guide our practice today. We also discuss a treatment algorith...

Effect of Lymphedema Treatment for Management of Acute Pilon Fractures.

Pilon fractures are high-energy injuries that often result in considerable edema and compromise of the soft tissue envelope of the ankle. These injuries are typically staged with an external fixator until the soft tissue is amenable for definitive fixation. This study was conducted to determine the effects of lymphedema treatment for the management of pilon fractures. Patients who underwent ope...

Optimal Management of High-Energy Pilon Fractures.

The ideal treatment method for fibular fractures associated with high-energy pilon injuries remains unknown. In this investigation, the authors reviewed a consecutive series of 137 patients who sustained high-energy pilon injuries and assessed the effect of fibular fixation and timing on radiographic and clinical outcomes. At a mean follow-up of 18.7 months, the authors found that the presence ...

Treatment of high-energy pilon fractures using the ILIZAROV treatment

The management of high-energy pilon fractures is still controversial. Open reduction and internal fixation are often associated with serious complications. Various methods have been used to treat these injuries, with variable results. The aim of this retrospective study was to analyze the clinical and radiographic outcome of the ILIZAROV technique in patients with high-energy pilon fractures. T...

Reduction strategies through the anterolateral exposure for fixation of type B and C pilon fractures.

The surgical management of pilon fractures has evolved over the last several years with treatment shifting from acute definitive fixation to delayed fixation. One of the driving forces behind this change was the high incidence of soft tissue complications in those patients with high-energy pilon fractures (Orthopaedic Trauma Association 43B and 43C) managed with acute stabilization. Meticulous ...
